Xylitol is silently present in every grocery aisle. It can be found in the low-calorie yogurt that bills itself as the healthy option, the sugar-free gum by the register, and the “natural” toothpaste at the drugstore. It had an almost unquestionable reputation for years—it was approved by regulators on both sides of the Atlantic, it was kinder than sugar, and it was better for your teeth. Last week in Munich, that reputation suffered a severe blow.
Researchers presented results from what seems to be the largest study of its kind to examine xylitol and cardiovascular outcomes at the annual congress of the European Society of Cardiology, the world’s largest heart conference. Under the direction of Dr. Marco Witkowski of Berlin’s Charité University Hospital, the team examined data from 17,710 participants selected from two sizable cohorts: one British and one Canadian. Even in a conference full of conflicting findings, the results were noteworthy enough to garner significant attention.

Compared to those with the lowest blood xylitol levels, those with the highest levels had a 57% higher chance of dying, having a heart attack, or having a stroke within six years. This figure represents the Canadian cohort over a period of six years. After being monitored for thirty years, the British cohort revealed an 18% increased risk. When combined, the two groups suggested a dose-dependent pattern. A higher rate of cardiovascular events is associated with higher blood levels of xylitol. Even after controlling for age, sex, BMI, diabetes, hypertension, and lipid levels, the association persisted.
Those changes are important, so it’s worth taking a moment to consider that. When lifestyle factors are taken into account, and a health association vanishes, it frequently indicates that the initial signal was noise. It didn’t vanish here. This is the aspect of the study that is more difficult to quickly rule out.
However, there is a crucial disclaimer that permeates the entire discussion. This study was observational in nature. It cannot prove that xylitol caused cardiac events, but it can demonstrate a statistical correlation between blood xylitol levels and those events. This was directly acknowledged by Dr. Witkowski.
The researchers also observed that elevated blood xylitol levels do not always indicate high xylitol consumption, which further complicates interpretation. Since the body naturally produces xylitol during the metabolism of glucose, the situation is actually more complicated than “you ate gum and now you’re at risk.”
The study does expand on previous research, also conducted by Witkowski’s group, which demonstrated that xylitol can increase platelet reactivity, thereby increasing the risk of blood clotting. The statistical result has some credibility because of this biological mechanism. It’s not an isolated, random correlation. Although more research is still needed to prove it, there is at least a working theory for how this might function.
Observing this discussion gives me the impression that the sweetener industry is facing a gradual reckoning that it most likely didn’t expect. Erythritol experienced similar clotting issues in 2023. Xylitol now. These products have gained attention as safer substitutes for sugar; both the companies and the regulators who approved them have subtly positioned them as such. However, the science was never as strong as the marketing claimed.
Although it is challenging to determine causality from observational data, as Prof. Dan Atar from the ESC Communication Committee pointed out, the results are important enough to warrant further investigation.
Whether this will alter regulatory guidance in the near future is still unknown. The belief that these sweeteners have undergone extensive testing for long-term cardiovascular safety is probably what needs to change. This belief is prevalent among consumers and possibly among some medical professionals. For the most part, they haven’t. This study aims to close the gap between widespread use and scant long-term evidence, and it is reasonable to do so.
For the time being, researchers’ and dietitians’ practical recommendations fall into a well-known, somewhat disappointing category: cut back on processed foods in general, don’t assume that any sweetener is safe, and don’t use this as an excuse to just substitute more sugar for xylitol. Neither way is that a green light. It’s simply the current location of the evidence.
